To schedule an appointment please call (408) 985-2020.

Please note that eye examinations may include a dilation. On average, for detailed examinations please allow 60-75 minutes for the visit. Please bring your glasses and contact lenses (contact lens container with name, power of contact lens, etc).

By giving you an appointment, we have reserved a time especially for you. Please kindly note that there will be a $45 charge for missed appointments without a 24 hour notification. Please bring your glasses and contact lenses (contact lens container with name, power of contact lens, etc).

Insurance Plans:

Our office participates with most insurance plans including PPO and Medicare with a secondary PPO plans, some EPO, and some POS plans. We do not work with any HMO plan. Please note that some plans contain both an HMO and PPO plan. We can then work with the PPO portion of your plan.

Please feel free to call our offices to see if your insurance plan is accepted.

Billing and Payment:

Co-payments and co-insurances are due at the time services are rendered. Forms of payment accepted by the office are check, cash, insurance assignment and major credit cards:

Telephone Calls to the Office:

If you have medical questions which only the doctor can answer, a message will be taken and your medical file will be given to the doctor who will return your call the same day. If your question is non-medical, a qualified staff member will be pleased to assist you.

Your Eye Examination:

Generally, examinations may include dilation of the eyes pupils to diagnose both general health problems such as diabetes, high blood pressure, etc. and local eye problems such as retinal detachment, and other eye problems.

Cancellations:

By giving you a specific appointment, we have reserved a time especially for you. Please kindly give us a 24 hour notification if you would like to cancel your appointment.
